CHAP Post Meeting Submissions
ADED-CHAP Post Meeting Submission

As a CHAP provider, you are required to furnish within 14 days of the events’ conclusion a post program submission via web portal to include: a complete registration list, sign in sheets, compilation of evaluations, a digital copy of any handouts and any additional marketing/promotional pieces not previously provided.
