 On the AMP website follow these instructions:

(1) Choose a Category select "Healthcare."
 
(2) Choose a Program select "Association for Driver Rehabilitation Specialists."
 
(3) Choose an Exam select "Driver Rehabilitation Specialist Certification Exam."
   
     

The deadline for returning applications for the 2016 exam is 7/15/2016.

 
Please note that the Application and Fees should be mailed to AMP.
